You are absolutely right that birthing on your back is not the best position atall. However, since 99.6% women in Israel choose to birth in hospital and not at home, about 95% of them birth on their back, since this is the accepted hospital birthing position. THe WORST position to birth in is a semi-sitting position, like in the fancy birth beds. This really closes your pelvis. The McRoberts position, where you are on your back with knees fully flexed and brought towards your chest, opens the pelvis much more. This is the advantage of the PushPal, as it helps you to pull your knees towards your chest. If someone else holds your legs for you, it is not the same as the movement that you pull your legs towards your stomach while you are pushing out. Of course, you can also use the PushPal for side birthing, and it is a wonderful aid for this postition too. If you squat or stand or birth on hands and knees, then you don't need it. Women who choose epidurals can really need the pushpal because they have a loss of feeling in their legs and can't raise them enough to grab them.
As for your other question about clotting disorders, it is best not to use anything that puts any pressure on any of the veins in the legs if you have clotting problems. You don't want anything to cause the blood to be "stagnant" in the veins. The same would go for any pressure from hosptial bed stirrips.
