Dear Zohar, A clinical trial was done to compare the effectiveness of the perineal massage and the epi-no. The results showed the epi-no to be much more effective than the perineal massage in reducing tearing at birth. I am a midwife of 20 years experience, and I hardly ever cut. For the past l8 years I have been suggesting to all my private women to do perineal massage, and my techniques at birth were basically the same: warm compresses, oil, slow deliverery of the head, keeping the head well-flexed, breathing during crowning. There was still tearing on first births, and quite a few needed stitching. Probably 25% of first births needed some stitches. Since I have been birthing women who are using the epi-no, I have noticed an amazing difference in the amount of tearing. These women stretch much easier, and very rarely do they need stitches. Also the pushing part is much faster and easier. So I am convinced that there is no need to do the perineal massage anymore, and it is really worth it to use the epi-no. The difference between 7-9 cm and 33-36 cm. relates to diameter verses circumference. An average baby has a head size of 10 cm. diameter and 34 cm. head circumference. Good luck and happy practicing.
